Smells like urine, no one at the desk to help. Very dirty and not very organized.

My son has been here for rehab following a stroke and other medical challenges for several months. The staff is communicative with me and has gone out of their way to accommodate his needs and comfort. The rehab is stellar. Sure, they are sometimes understaffed, but that is the standard for nursing homes these days. I am so appreciative of the excellent staff from the admissions manager to the CNAs and everyone in between. Thank you.
Genie at Evergreen gave truly outstanding care. She combined professionalism with genuine compassion, making a difficult experience much easier. I’m grateful for the respect, comfort, and kindness she showed throughout.

Evergreen Hospital has been such a blessing for me and my family. The care and services they provide are truly outstanding, and every staff member shows compassion and professionalism. I feel safe and well cared for here, and I’m grateful for everything they’ve done for my health and my loved ones. Thank you for being such a good hospital and always putting patients first.
Many of the staff at this place are horrible. My brother is terminal, he is on hospice. He had a bed wetting accident during the night. He asked the staff if they could change his bedding. They refused. He had to sleep on the floor from midnight until the day shift came in. That is elder abuse, inhumane and disgusting. I wouldn’t treat my animal like that. He has dietary restrictions that they don’t bother to follow. They serve him food he can’t eat without getting sick. He’s talked to them time and again, I’ve talked to them and still it happens. I don’t know how this facility stays in business. Luckily my brother only has to put up with it for a few more days, he is moving to another facility.
Wow. Do not send your loved one here if you can help it. The staff are nice enough, but the experience is not very conducive to healing. It is very loud, 3 ppl to a room, not very clean. It is like a zoo. There are significant electrical challenges (see photo). There are 3 TV’s in each room (one for each resident) and the remotes all control the other TV’s, when they work. This place should not get more than 2 stars from anyone… geez.

Hi Gennie, Thank you for The nursing and therapy teams are outstanding. Whether it’s physical, occupational, or speech therapy, the personalized attention and dedication to recovery are evident. Residents receive care that’s not just clinically sound but emotionally supportive too
The staff comradery really helps the facility feel amazing. I loved my time here at evergreen post acute. I git great rehab here. The therapist all care a lot and take the time to help me improve. I broke my arm and the therapists have helped me regain strength and mobility. I loved the group activities. Many residents are involved and we all got to get to know ow each other that way. I formed some friendships here which I wasn’t anticipating in this setting. There is a lot of compassion in this building.

It’s not a perfect world so it’s not a perfect place but I give 5 stars because they try really hard. The food is edible but the nutritionist was at my bed on day one asking what I needed. 5 stars for that. I get my medication on time and that is very important to me but I also am very persistent and sometimes go to the med cart to wait and they don’t seem to mind. One other thing is they have an activities director named Sara who is very good at her job and very earnest. On Father’s day she and Pearl put out white tablecloths and served coffee and donuts to the fathers first and then to everyone else. Then they gave all the men baseball hats! Again it’s not perfect but the nurses and CMAs work long shifts and they run from the time they start until they clock out. I want to give a shout out to Dacian, Donna, Sandy, Gladwell and others whose names I can’t remember but totally respect. This place just like in life is what you make it. I’ve been a resident at Evergreen post acute for 5 years. Since the new ownership things here have improved immensely. I love the new Administrator, Russell. He always makes sure I have what I need and makes me feel at home. Emily the social worker is the best, Tracy the DNS always has time to talk to me. The staff are so welcoming. I love the new dining room, the lighting and getting my food in there is great. Since the new dining room has been done we have tons of residents eating in there. It’s helped it feel like a real community. The renovation is so awesome. It’s given new life to this place. I love the staff here, they are great! The new pavement is great! I can come and go with ease in my wheelchair. I would recommend anyone to come here. Evergreen is like a big family.
The night staff here are very disrespectful, mean,rude,it’s. Like the just don’t care about anything,and as a patient here I have said something a few times and nobody even checks in to it. Now the day staff there are like3 that are amazing people but I have asked for somethings that everyone else get but for some reason I keep getting put off, like being able to clip my finger nails shave,ect. And TV right now I have to pay to watch TV I have no money 3weeks ago they said they would hook up a Comcast box like everyone else’s TV. Now I’m paralyzed so I can’t really do all that much for myself. It don’t really Mayer because nobody ever believes me or people like me so yeah thanks for reading this.
It’s not what you think
My mom stayed here, and it was the best experience!! The administration was amazing and the food was so good and she was so well taken care of. I was so happy about her care. I would recommend this facility to anyone
This place is top notch! Super impressed with the new management and level of care they provide. This facility is very clean and welcoming!
The staff here are so amazing. I was also really impressed by the new renovations – it feels really fresh and clean. They also have a good variety of activities to offer which was very helpful. I definitely recommend checking out this place.
This place is really nice! They’ve got new management, new renovations, great activities and the food is pretty good!
My grandma is a veteran and she’s pretty picky. It was really hard to find a place that she liked. We looked at many facilites but this facility is very veteran friendly, they even had a big Veterans Day breakfast for all the residents. Grandma felt so special at this breakfast.
